I'm almost finished reading "How Customers Think" by Gerald Zaltman. This book describes the methodology you list above, plus a lot more. It's a bit creepy, digging around in customers' psyches, but apparently effective. The author caveats all this by reminding marketers to use their conscience and keep the customers' best interests in mind.
